Have you tried Allen Carr's book to stop smoking? I find him the most infuriating man but if you can deal with all the bullsh*t it is a remarkably effective book. I tried the short version and failed then bought the long one and read it over several months (including lying on a beach in Sydney, now that's dedication!) and when I finally put out my last cigarette it was incredible - the next time I thought about smoking was three days later. It's rather effective as one continues to smoke throughout the book and it's only at the very end that the last cigarette is smoked and so it's totally at your speed - if you only read one page a week, fine, or whizz through it, fine. Doing it this way also takes all the fright out of it and the insecurity. I didn't put on weight and very soon felt the benefit - however, this has to be your decision when to do it and how to do it but you don't have to suffer and Carr does remove the suffering. Great list btw, sorry to harp on the one bit but it is the one I know about!
